VIRGINIA
Arlington,VA
Arlington VOA Assisted-Living

The Arlington VOA ALCP application proposes to convert an unused/underutilized commercial facility into a fully handicapped accessible 52-unit assisted living facility. The renovation of this property will provide emergency response systems including 24 hour a day on-site staff to respond to the residents' needs, at least one cardiopulmonary resuscitation (CPR) certified staff member and emergency pull cords in all bedrooms and bathrooms. Other features to ensure the residents' safety against fire include duct smoke detectors and a sprinkler system.

The Arlington VOA Supportive Services Plan (SSP) includes provisions for three meals and two snacks per day based on individual dietary needs, house keeping, personal care services, counseling services, social activities and the administration of medication under the general supervision of a nurse. The SSP documents financial support from the Department of Human Services to provide an auxiliary grant to enable the residents to pay the established assisted living facility rate. It is anticipated that all of the residents will qualify for this grant funding. DHS also provided a written commitment to provide $880,914, should the Arlington VOA successfully compete for the Fiscal Year 2005 ALCP funds.

The funds will be used for the conversion of 15 elderly units to assisted living units for the frail elderly. Conversion activities consist of reconfiguration of the units, refurbishing of the elevator to meet accessibility requirements; renovation of the kitchen; dining room and common space for accessibility. Funds will also be used to enhance the lighting in the common area including hallways, upgrade hand rails, painting, upgrading of laundry area and the upgrading of the front entrance doors to meet accessibility requirements.

The elderly residents will be provided support services through a multitude of community agencies and the State of Pennsylvania. The Assisted Living Conversion grant will allow Guild House West I to achieve their mission to provide quality, affordable care to elders, maximizing their independence within a home like environment, with the goal of avoiding premature and unnecessary institutionalization. The project currently has a Service Coordinator.

Services provided include congregate meals, visiting nurses, dental care, housekeeping, transportation, dementia care, and emergency response. Costs include relocation expenses, and rehabilitation, including wider doors, wheelchair accessible bathrooms with grab bars, minimum 30" front approach to lavatories, showers, roll-in and transfer, with grab bars; kitchens with wheelchair accessible work surface and sink counters; common area opposite elevators with laundry area, with accessible kitchen, dining area, recreation, social and seating area.

Services provided include congregate meals, visiting nurses, dental care, housekeeping, transportation, dementia care, and emergency response. Costs include relocation expenses, and rehabilitation, including wider doors, wheelchair accessible bathrooms with grab bars, minimum 30" front approach to lavatories, showers, roll-in and transfer, with grab bars; kitchens with wheelchair accessible work surface and sink counters; common area opposite elevators with laundry area, with accessible kitchen, dining area, recreation, social and seating area.

Shelton, CT
Wesley Heights III $1,585,501.00


Funds will be used to convert 30 units into a fully accessible Assisted Living Facility in Shelton, CT, for frail elderly persons, with common and resident service space on the floor. Renovate the activity area, employee dining room, and employee restrooms. Install an elevator to improve access between the main floor where the apartments, dining room, and kitchen are located and the lower level where the business and other administrative offices are located and where food deliveries are received.


MASSACHUSETTS
Hubbardston, MA
Hubbardston Elderly Housing, I
$1,373,409.00


Hubbardston Elderly Housing is a two-story walk-up, 36-unit Section 202 Direct Loan Project in Hubbardston, MA. This grant will convert 7 units in the North Wing of the Project into fully accessible assisted living units, and thus will further the Departmental priority of allowing elders to "age-in-place". The grant will also fund the addition of an elevator to the project, an essential component for any assisted living environment.


MASSACHUSETTS
Hubbardston, MA
Hubbardston Elderly Housing, II
$1,226,709.00


Under phase II, grant funds will be used to convert 12 units in the North Wing of the Hubbardston Elderly Housing project into fully accessible assisted living units, and thus will further the Departmental priority of allowing elders to "age-in-place". The grant will also fund the addition of a sprinkler system to the project, an essential component for any assisted living environment and necessary for Code compliance.

The Assisted Living Conversion Program grant funds will be used to renovate 10 elderly housing units on the 10th floor of this existing 24-story building into 9 fully accessible units for the frail elderly and one laundry room that will serve the residents of floors 8, 9 and 10. The unit conversions will facilitate the delivery of services in an economic manner and accommodate the changing needs of the residents over the next decade.

FLORIDA
Clearwater, FL
Prospect Towers
$3,343,098.00


Prospect Towers is a 17 story 208-unit building built in 1972 and financed through the HUD Section 236 program. The project plans to convert twenty-four (24) existing elderly units to assisted living facility units. They have prior experience with the ALCP with Palm Avenue Baptist Towers. Conversion will include renovation of the unit kitchens, bathrooms, and common areas improvements which will be compliant with ADA requirements and conform to UFAS. Safety features will include video surveillance and new/upgraded fire alarm system and wireless emergency call system. The wireless communication call system was considered most effective with the Palm Avenue conversion. All renovations slated for the conversion will meet energy star standards and current energy efficiency standards.
