You do not Need To Fill Out Separate Enrollment Documents For Each Facility You Operate. If you are an MFU and would like to enroll additional facilities, please print the Wholly Owned Addendum. Simply list the names and addresses of all facilities you’d like to include under your enrollment, and submit the signed addendum along with your completed enrollment documents.
(Note: If it is more convenient, you may also attach a list of facilities to the signed Addendum.)
